Work Schedule

Monday – Friday, 8:30 AM – 4:30 PM

Role Summary

We are currently seeking a full-time Client Support Specialist for our Home Safe program. The Specialist will engage directly with target populations, ensuring the completion of necessary program documentation and tracking of milestones and outcomes. Responsibilities include documenting all client interactions, collaborating within an interdisciplinary team, and maintaining a thorough understanding of community resources. The Specialist will also participate in agency-wide systems of care and collaborative projects with external partners.

Key Responsibilities

  • Conduct linkage, coordination, advocacy, mentoring, and skill-building activities.
  • Provide anger management and parenting skills training to youth and their families.
  • Focus on goal achievement related to factors contributing to youth involvement with mental health services.
  • Deliver case management and skill-building services to youth enrolled in the program.
  • Perform home visits throughout the designated service area.
  • Complete all other assigned duties as required.

Qualifications

A Bachelor's degree in social work or a related field is preferred. Candidates should have at least 4 years of experience working with families in a similar capacity, or a Bachelor's Degree with 2 years of relevant experience. The ideal candidate will demonstrate the ability to engage with diverse populations and effectively involve both youth and parents in service delivery.

Strong interpersonal, verbal, and written communication skills are essential. Candidates must possess knowledge of community resources, demonstrate sound judgment, and exhibit reliability and flexibility. Proficiency in computer skills, including Microsoft Office, is required. A valid and clean NYS Driver's License is necessary, and bilingual proficiency in Spanish is preferred.

Physical Requirements

  • This position involves prolonged periods of sitting at a computer and traveling for home visits.
