Position Overview

Work Schedule

Monday – Friday, 8:30 AM – 4:30 PM

Role Summary

We are currently seeking a full-time Client Services Coordinator for our Home Safe initiative in Bay Shore. The Coordinator will provide direct assistance to targeted groups, ensuring thorough documentation of program activities and tracking of milestones and outcomes. Responsibilities include maintaining detailed records of client interactions, collaborating within an interdisciplinary team, and staying informed about available community resources. The Coordinator will also engage in agency-wide care systems and collaborative projects with external partners.

Key Responsibilities

  • Facilitate linkage, coordination, advocacy, mentoring, and skill development.
  • Deliver anger management and parenting skills training to youth and their families.
  • Focus on achieving goals related to the factors contributing to youth engagement with mental health services.
  • Provide case management and skill-building services to youth enrolled in the program.
  • Conduct home visits throughout the designated service area.
  • Perform additional duties as assigned.

Qualifications

A Bachelor's degree in social work or a related field is preferred.

Minimum of 4 years of experience working with families in a relevant capacity, or a Bachelor's Degree with 2 years of experience.

Ability to work effectively with diverse populations and engage both youth and parents in service delivery is essential.

Strong interpersonal, verbal, and written communication skills are required.

Knowledge of community resources, sound judgment, reliability, flexibility, and multitasking abilities are crucial.

Proficiency in computer applications, including Microsoft Office, is necessary.

A valid and clean NYS Driver's License is required.

Bilingual proficiency in Spanish is preferred.

Physical Requirements

  • This role involves prolonged periods of sitting at a computer and traveling for home visits.
