Job Summary

The Certified Diabetic Educator provides high-quality, comprehensive care for diabetic patients through individual consults and classes, interacting with other team members in planning and delivering appropriate patient care. This role serves as a resource for physicians and other personnel, requiring a well-organized individual with excellent communication, listening, motivation, education, problem-solving, and counseling skills.

Key Responsibilities

  • Under the clinical direction of the Assistant Director of Clinical Coordination, conducts individual assessments and works with family/support persons of the diabetic patient.
  • Develops a plan of care for patients referred to the Diabetes Management Program, acting as a resource for professional staff and collaborating in the development of a plan of care for diabetics in other service areas of the VNA.
  • Develops and implements individual and group educational sessions to meet the assessed needs of patients according to the American Diabetes Association guidelines, serving as an educator to assist patients and/or family and interdisciplinary teams to understand diabetes treatment needs.
  • Serves as a liaison between the interdisciplinary diabetes team members and patients, consulting with referring MDs to discuss difficult diabetes cases, facilitating communication between team members and patients, and integrating information regarding patient needs.
  • Provides clinical expertise by understanding diabetes treatment plans, goals, and objectives of the Diabetic Collaborative Project, actively involved in health prevention, primarily weight management, exercise, and proper nutrition, establishing and maintaining links to diabetes care resources.
  • Provides patient education on diabetes self-management and identifies varied opportunities for teaching.
