Overview

At Brookdale Vancouver Stonebridge, we're committed to providing exceptional care and services to our residents. As a Resident Care Coordinator, you'll play a vital role in ensuring the well-being and dignity of our residents.

Responsibilities
  • Provide direct supervision of care giving within the community, ensuring residents are treated with respect and dignity.
  • Monitor job performance, collaborate with community leadership on associate performance evaluations, and conduct coaching and counseling as needed.
  • Coordinate training and education of resident care staff on an ongoing basis.
  • Verify associate hours on time cards and weekly hours report, complete nursing schedules, and arrange replacement staffing when necessary.
  • Observe residents' health status, take actions to address concerns within scope of practice, and report significant changes to the Health and Wellness Director.
  • Assist with medication management, ordering medication from pharmacy, and maintaining records according to Brookdale Senior Living policies and procedures.
  • Monitor staff in administration of medication, conduct audits of medication administration records, and respond promptly to emergency calls from residents.
  • Communicate with families regarding residents' personal needs and achievements, remind families of scheduled appointments, and assign vital signs to associates.
  • Participate in department quality improvement activities, complete quality assurance surveys, and perform daily, weekly, and monthly safety and infection control checks.
  • Share on-call duties as required and participate in community functions, including family socials and open houses.
