The Patient Records Coordinator plays a crucial role in ensuring that all patient documentation and supplementary materials are accurately digitized into the Horizon Patient/Business Folder, maintaining the highest standards of quality.

This role involves the systematic indexing of all documents to their respective patient folders, ensuring that each document is correctly labeled with the appropriate title.

Additionally, the Coordinator is tasked with verifying the completeness of patient charts and overseeing the reanalysis queue to confirm that all records and documents are processed correctly.

Maintaining the master patient index is also a key responsibility, which includes merging duplicate patient entries, reallocating misfiled visits, and correcting document assignments.

The Patient Records Coordinator is responsible for gathering, maintaining, and reporting data on incomplete physician records to support timely coding, monitor record completion, and provide essential information for quality assurance and physician credentialing.

Key Responsibilities:

  • Document Preparation and Scanning
1.1 Collects all records of discharged patients from various units and interoffice communications on a daily basis by 10 a.m.

Qualifications:

EDUCATION
A high school diploma or equivalent is required; an Associate's degree is preferred.

EXPERIENCE
A minimum of 1 year of experience in medical records/document imaging is required, with a preference for 1 year of record analysis experience. Familiarity with electronic medical record systems is advantageous.

LICENSURE
No specific licensure is required.
