The Record Analyst is responsible for assuring that all patient records and loose documents are scanned into Horizon Patient/Business Folder with the highest level of quality possible. The Record Analyst is also responsible for indexing all documents to the appropriate patient folder and for ensuring each document is assigned the appropriate document name. This position will also be responsible for verifying chart completion and monitoring the reanalysis queue to ensure all records/documents are completed appropriately. The Record Analyst is responsible for maintaining the master patient index by merging duplicate patients, moving incorrectly assigned visits, and moving incorrectly assigned documents. The Record Analyst, collects, maintains, and reports physician incomplete record data to facilitate timely coding, monitor record completion status, and provide information for quality assurance and physician credentialing.

Responsibilities:
  • 1. Document Prep/Scanning
    • 1.1 Retrieves all records of discharged patients from all units and interoffice mail once per day by 10 a.m.


Qualifications:

EDUCATION

High school diploma or equivalent. AS degree preferred.

EXPERIENCE

1 year medical records/document imaging experience required. 1 year record analysis experience preferred. Experience with an electronic medical record system preferred.
