Job DescriptionPOSITION OVERVIEW FOR CASE MANAGER - MANAGEMENT ANALYST I
Location: Wilmington Delaware
Schedule: Monday - Friday 8am to 4:30pm
Compensation: $16.79 per hour

ROLE RESPONSIBILITIES:
  1. Conduct interviews with applicants, beneficiaries, family members, and other stakeholders to gather and share both general and sensitive information, aimed at assessing eligibility and facilitating effective referrals. Evaluate client requirements based on the information collected and manage referral processes.
  2. Accurately assess and re-evaluate eligibility for various Division of Social Services programs, including TANF, GA, Medical, Child Care, and Food Benefits.
  3. Utilize computerized systems for data entry and management across all relevant information platforms, including DCIS, DOL, MTRV, MMIS, Delvax, EVRS, DCSE, DIS, among others.
  4. Facilitate referrals to contractors and vendors while monitoring participant adherence through alerts, case notes, and electronic communications in accordance with the Contract of Mutual Responsibility (CMR), implementing sanctions as necessary.
  5. Provide voter registration opportunities during each application, re-evaluation, and recertification process, managing the associated registration forms or data entry through the voter registration system.
  6. Employ the Family Development Profile to evaluate self-esteem and identify barriers to self-sufficiency within TANF.
  7. Formulate and revise the Contract of Mutual Responsibility (CMR) to underscore work and parental obligations during each review period related to TANF.

QUALIFICATIONS:
Applicants should possess education, training, or experience that demonstrates proficiency in the following areas:
  1. A minimum of 6 months of experience in Case Management.
  2. Familiarity with the principles, practices, methods, and techniques associated with social work.
  3. Proficiency in interviewing applicants and analyzing, assessing, and identifying their needs.
  4. Ability to establish effective collaborative relationships with colleagues and clients.
  5. Strong narrative writing capabilities.
  6. Proficiency in Microsoft Office Suite, particularly Word and Excel.
