Key Responsibilities
- Conduct thorough assessments of individuals' needs and develop personalized service plans to meet their goals.
- Coordinate and provide support services, including health and wellness, employment, and social activities.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care and support.
- Monitor and evaluate the effectiveness of service plans and make adjustments as needed.
- Maintain accurate and up-to-date records of client interactions and service delivery.
Requirements
- Minimum of one year of experience in health or human services support, including interviewing individuals and assessing their needs.
- Six months of experience in case management, including assessing, planning, and implementing services to meet individuals' needs.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Associate's degree or higher in a related field, such as social work or healthcare administration.
Preferred Qualifications
- Bilingual (Spanish) proficiency.
- Experience working with individuals with intellectual challenges and developmental disabilities.
