As a key member of our case management function, you will provide critical support to clients, including transportation, appointment scheduling, and coaching. Your exceptional communication and interpersonal skills will enable you to build strong relationships with clients, colleagues, and community partners.

Key Responsibilities

  • Schedule appointments for clients and transport them to required locations, providing coaching and support to achieve desired outcomes.
  • Provide office and telephone coverage in the absence of the case manager, referring complex issues to the appropriate party.
  • Visit clients in various environments, such as homes, schools, and community settings, to observe behavior and report to the case manager.
  • Prepare and maintain accurate records and forms required by the case manager to deliver high-quality services to clients.
  • Monitor client behavior during parental visits, ensuring safe interactions and reporting concerns to the case manager.
  • Interview clients to obtain basic information, such as dates of birth and social security numbers, as required.

Requirements

To be successful in this role, you must have education, training, and/or experience demonstrating competence in the following areas:

  • Coursework, training, or work experience in health or human services support, including interviewing clients and assessing personal, health, social, or financial needs, as well as coordinating with community resources to obtain client services.
