Position Overview

We are currently seeking a full-time Client Care Coordinator for our Adult Care Management State Funded program. This role is essential in addressing the complex needs of our clients while adhering to the visit requirements set forth by the Office of Mental Health (OMH). Our clientele primarily consists of individuals diagnosed with serious mental health conditions, along with potential co-occurring medical and substance use disorders.

Key Responsibilities

  • Maintain effective working relationships with Suffolk County Adult SPOA.
  • Utilize a culturally competent and proactive outreach strategy to engage potential clients.
  • Manage a caseload of 25-35 clients, ensuring their comprehensive needs are met.
  • Fulfill minimum visit requirements based on the acuity and needs of clients.
  • Collaborate with mental health, medical, and substance abuse providers, as well as hospitals and shelters, as necessary.
  • Establish and sustain professional relationships with both internal and external service providers to coordinate care effectively.
  • Complete all necessary documentation promptly and accurately.
  • Report incidents to program management as required.
  • Adhere to all agency and program mandates established by Family Service League, the Department of Health, and Suffolk County Division of Mental Hygiene.
  • Perform additional duties as assigned.

Qualifications

A Bachelor's Degree in a health-related field is required.

A minimum of 2 years of experience providing direct services to individuals with serious mental health issues is essential.

Proficiency in Microsoft Office is required; experience with Electronic Health Records (EHR) is preferred.

Strong skills in assessment, engagement, organization, and time management are necessary.

Excellent verbal and written communication abilities are required.

A valid and clean New York State Driver's License is mandatory.

Physical Requirements

This position frequently involves sitting and working at a computer for extended periods, as well as traveling to various locations for community meetings and home visits.
