Position Overview

We are currently seeking a full-time Case Manager for our Adult Care Management State Funded program. This role is essential in addressing the complex needs of our clients, ensuring compliance with visit requirements as mandated by the Office of Mental Health (OMH). Our clientele primarily consists of individuals diagnosed with serious mental health conditions, along with potential co-occurring medical and substance use disorders.

Key Responsibilities

  • Establish and maintain a collaborative relationship with Suffolk County Adult SPOA.
  • Utilize a culturally sensitive and proactive outreach strategy to engage potential clients.
  • Manage a caseload of 25-35 clients, addressing their intensive needs effectively.
  • Fulfill minimum visit requirements based on the acuity level and needs of clients.
  • Communicate with mental health, medical, and substance abuse providers, as well as hospitals and shelters when necessary.
  • Develop and sustain positive professional relationships with both internal and external service providers to coordinate care and gather essential information.
  • Complete all necessary documentation accurately and in a timely manner.
  • Report incidents to program management as required.
  • Adhere to all agency and program guidelines established by Family Service League, the Department of Health, and Suffolk County Division of Mental Hygiene.
  • Perform additional duties as assigned.

Qualifications

A Bachelor's Degree in a health-related field is required, along with a minimum of 2 years of experience in providing direct services to individuals with serious mental illness. Proficiency in Microsoft Office is essential, and experience with Electronic Health Records (EHR) is preferred. Strong skills in assessment, engagement, organization, and time management are crucial, along with excellent verbal and written communication abilities. A valid and clean NYS Driver's License is also required.

Physical Requirements

This position often necessitates prolonged periods of sitting and working at a computer, as well as travel to various offices, community meetings, and home visits.
