Position Overview

WORK SCHEDULE:

Monday to Friday, 9 AM to 5 PM

POSITION SUMMARY:

Family Service League Inc. is in search of a dedicated full-time Client Support Specialist for our Assertive Community Treatment (ACT) initiative. The Client Support Specialist will engage in home visits and deliver educational resources to clients. This role also involves developing treatment strategies and linking clients to appropriate care facilities.

KEY RESPONSIBILITIES:

  • Assist clients in completing Entitlement Applications.
  • Accompany clients to Entitlement Offices as needed.
  • Provide guidance on financial management and budgeting.
  • Ensure clients maintain active entitlements, including Medicaid and other benefits.
  • Deliver psycho-educational support to families and clients.
  • Facilitate group sessions.
  • Conduct client assessments utilizing relapse prevention strategies.
  • Draft treatment plans tailored to client needs.
  • Employ motivational interviewing techniques.
  • Connect clients with treatment facilities.
  • Communicate with managed care organizations.
  • Assist clients in applying for benefits (e.g., Medicaid, SSI/SSD, HEAP, Food Stamps).
  • Provide crisis intervention services.
  • Respond to crisis phone calls.
  • Participate in daily team meetings and case discussions.
  • Conduct regular home visits.
  • Perform additional duties as assigned.

QUALIFICATIONS:

A High School Diploma or equivalent is required; a bachelor's degree is preferred.

Minimum of one year of experience working with individuals experiencing severe and persistent mental illness (SPMI) and familiarity with community resources such as DSS, SSI, Medicaid Managed Care, and Housing is essential.

Strong organizational and interpersonal skills are required.

Excellent verbal and written communication skills are necessary.

Ability to manage multiple tasks and prioritize effectively in a dynamic environment.

Proficient in computer applications, including Microsoft Office.

A valid and clean New York State Driver's License is required.

PHYSICAL DEMANDS:

This role necessitates prolonged periods of sitting and travel for home visits. The ability to lift up to 10 lbs is required.
