Position Overview

We are currently seeking a full-time Case Manager for our Adult Care Management State Funded program. This role is essential in addressing the complex needs of our clients, ensuring compliance with visit requirements set forth by the Office of Mental Health (OMH). Our clientele primarily consists of individuals diagnosed with serious mental health conditions, along with potential co-occurring medical and substance use disorders.

Key Responsibilities

  • Establish and maintain effective working relationships with Suffolk County Adult SPOA.
  • Utilize a culturally sensitive and proactive outreach strategy to engage potential clients.
  • Manage a caseload of 25-35 clients, addressing their intensive needs.
  • Adhere to minimum visit requirements based on the client's assessed acuity and needs.
  • Collaborate with mental health, medical, and substance abuse providers, as well as hospitals, jails, and housing/shelter organizations when necessary.
  • Foster and sustain positive professional relationships with both internal and external providers to coordinate care effectively.
  • Complete all necessary documentation promptly and accurately.
  • Report incidents to program management as required.
  • Comply with all agency and program mandates as established by Family Service League, the Department of Health, and the Suffolk County Division of Mental Hygiene.
  • Perform additional duties as assigned.

Qualifications

A Bachelor's Degree in a health-related field is required.

A minimum of 2 years of experience providing direct services to individuals with serious mental health issues is essential.

Proficiency in Microsoft Office is required; experience with Electronic Health Records (EHR) is preferred.

Strong skills in assessment, engagement, organization, and time management are necessary.

Excellent verbal and written communication abilities are required.

A valid and clean New York State Driver's License is mandatory.

Physical Requirements

This position may involve extended periods of sitting and working at a computer, as well as traveling to various Family Service League offices, community meetings, and conducting home visits.
