Nurse Practitioner Resident


As a Nurse Practitioner, you will deliver comprehensive primary health care services to individuals and families, focusing on health promotion and the prevention of diseases, while facilitating referrals to physicians and other members of the healthcare team as necessary.

This role involves close collaboration with clinical staff and reporting to the Site Medical Director and Site Operations Director.


KEY RESPONSIBILITIES:

  • Develops and implements effective patient care strategies.
  • Ensures continuity of care for patients.
  • Provides holistic care, including referrals to additional healthcare providers.
  • Maintains thorough, clear, and accurate documentation of patient interactions.
  • Conducts and records appropriate periodic health evaluations.
  • Offers relevant patient education.
  • Exhibits professional work habits, including punctuality, timely communication, adherence to universal precautions, and maintaining patient confidentiality.
  • Achieves a minimum of 2946 outpatient visits with patients annually.
  • Engages in assigned Quality Assurance committees.
  • Regularly participates in Team/Clinical meetings.
  • Takes part in nurse practitioner performance evaluations as required.
  • Aids in the formulation and upkeep of practice guidelines as designated.
  • Contributes to the QA Committee as assigned.
  • Attends mandatory in-service training sessions.
  • Delivers community presentations upon request.
  • Represents the provider staff at relevant boards or local organizations when needed.
  • Instructs students as assigned.
  • Participates in annual continuing education sessions as required for certification.
  • Attends regional or national meetings for Community Health Centers as requested.
  • All HealthLinc staff are dedicated to engaging in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH) principles.
  • Performs additional duties as assigned.
ESSENTIAL QUALIFICATIONS:
Education/Training

  • Graduate from an accredited nursing and nurse practitioner program.
  • Master’s degree in Nursing.
  • Valid Indiana registered nurse license.
  • Certification as a nurse practitioner from an accredited certifying body.
  • Prescriptive authority and DEA certification.
  • Current CPR certification.
Experience

  • Minimum of 1-3 years of experience as a Nurse Practitioner or substantial experience as an RN in emergency or intensive care settings.

  • Excellent customer service abilities.
  • Strong organizational and time management skills.
  • Proven capacity to work effectively within a team.
  • Exceptional written and verbal communication skills.
  • Ability to remain flexible and adaptable to changing situations.
  • Bilingual in Spanish is preferred.
  • Possession of reliable transportation and a valid driver’s license.
  • Capability to adhere to HealthLinc policies and procedures.
TECHNOLOGY SKILLS:
  • Proficient in operating multi-line phone systems and office equipment such as printers and fax machines.
  • Basic knowledge of software applications (Microsoft Office, EHR, online resources, etc.).

DIRECT SUPERVISION:
  • N/A

REQUIRED TRAINING:
  • Completion of all assigned Relias training.
