Nurse Practitioner Resident


As a Nurse Practitioner, you will deliver comprehensive primary healthcare services to individuals and families, focusing on health promotion and disease prevention while coordinating referrals to physicians and other healthcare professionals as necessary.

This role involves close collaboration with clinical personnel and requires reporting to the Site Medical Director and Site Operations Director.


KEY RESPONSIBILITIES:

  • Develops and implements effective patient care strategies.
  • Ensures continuity of care for patients.
  • Provides holistic care, including referrals to other healthcare specialists.
  • Maintains thorough, clear, and accurate documentation of patient interactions.
  • Conducts and records periodic health evaluations.
  • Delivers relevant patient education.
  • Exhibits professional work habits, including punctuality, timely communication, adherence to universal precautions, and maintaining patient confidentiality.
  • Completes a minimum of 2946 outpatient consultations annually.
  • Participates in designated Quality Assurance committees.
  • Regularly attends clinical team meetings.
  • Engages in performance audits for nurse practitioners as assigned.
  • Contributes to the formulation and upkeep of practice guidelines as needed.
  • Participates in Quality Assurance Committee activities as assigned.
  • Attends mandatory in-service training sessions.
  • Delivers community presentations when requested.
  • Represents the nursing staff at organizational meetings if needed.
  • Instructs students as assigned.
  • Completes annual continuing education requirements for certification.
  • Attends regional or national meetings relevant to Community Health Centers as requested.
  • All HealthLinc staff are dedicated to participating in quality improvement initiatives that align with Patient-Centered Medical Home (PCMH) principles.
  • Performs additional duties as required.
QUALIFICATIONS:

EDUCATION AND TRAINING:
  • Graduation from an accredited nursing and nurse practitioner program.
  • Master's degree in Nursing.
  • Current registered nurse license in Indiana.
  • Certification as a nurse practitioner from an accredited certifying body.
  • Possession of prescriptive authority and DEA certification.
  • Current CPR certification.
EXPERIENCE:
  • 1-3 years of experience as a Nurse Practitioner or substantial experience as a Registered Nurse in emergency or intensive care settings.
SKILLS AND REQUIREMENTS:
  • Exceptional customer service abilities.
  • Strong organizational and time management skills.
  • Proven capability to collaborate effectively within a team.
  • Excellent written and verbal communication skills.
  • Ability to adapt and remain flexible in various situations.
  • Bilingual in Spanish is preferred.
  • Possession of a valid driver's license and reliable transportation.
  • Ability to adhere to HealthLinc policies and procedures.
TECHNOLOGY SKILLS:
  • Proficient in operating multi-line phone systems and office equipment such as printers and fax machines.
  • Basic knowledge of software applications (Microsoft Office, EHR systems, online resources, etc.).

  • N/A

TRAINING REQUIREMENTS:
  • Completion of all assigned Relias training modules.
