Nurse Practitioner Resident


As a Nurse Practitioner, you will deliver comprehensive health care services to individuals and families, focusing on health promotion and disease prevention while coordinating referrals to physicians and other healthcare professionals as necessary.

This role requires close collaboration with the clinical team and will be under the supervision of the Site Medical Director and Site Operations Director.


KEY RESPONSIBILITIES:

  • Develops and implements effective patient care strategies
  • Ensures continuity of care for patients
  • Provides holistic care, including referrals to additional healthcare providers.
  • Maintains thorough, clear, and accurate documentation of patient interactions.
  • Conducts and records periodic health evaluations.
  • Offers relevant patient education.
  • Exhibits professional work habits such as punctuality, timely communication, adherence to universal precautions, and maintaining patient confidentiality.
  • Completes a minimum of 2946 outpatient consultations annually.
  • Engages in assigned Quality Assurance initiatives.
  • Actively participates in Team/Clinical meetings.
  • Involved in nurse practitioner performance evaluations as designated.
  • Aids in the creation and upkeep of practice guidelines as required.
  • Participates in the QA Committee as needed.
  • Attends mandatory in-service training sessions.
  • Represents the organization at community presentations when necessary.
  • Serves as a representative for the provider staff at local organizations if requested.
  • Instructs students as assigned.
  • Attends annual continuing education sessions as required for certification.
  • Participates in regional or national meetings related to Community Health Centers as requested.
  • All staff members are dedicated to engaging in quality improvement efforts that align with Patient-Centered Medical Home (PCMH) principles.
  • Performs additional duties as assigned.
QUALIFICATIONS:

EDUCATION AND TRAINING:
  • Graduate from an accredited nursing and nurse practitioner program
  • Master’s degree in Nursing
  • Valid registered nurse license in Indiana
  • Certification as a nurse practitioner from an accredited certifying body.
  • Prescriptive authority and DEA certification
  • Current CPR certification
EXPERIENCE:
  • Minimum of 1-3 years as a Nurse Practitioner or substantial experience as an RN in emergency or intensive care settings
SKILLS:
  • Excellent customer service abilities
  • Strong organizational and time management capabilities
  • Proven teamwork skills
  • Exceptional written and verbal communication
  • Ability to adapt and remain flexible
  • Bilingual in Spanish is preferred
  • Valid driver’s license and reliable transportation
  • Commitment to following organizational policies and procedures
TECHNOLOGY SKILLS:
  • Proficient in operating multi-line phone systems and office equipment
  • Basic knowledge of software applications (Microsoft Office, EHR, online resources, etc.)

  • N/A

TRAINING:
  • Completion of all assigned Relias training
