Nurse Practitioner Resident


As a Nurse Practitioner, you will deliver comprehensive health services to individuals and families, focusing on health enhancement and illness prevention, while coordinating referrals to physicians and other healthcare professionals as necessary.

This role involves close collaboration with the clinical team and direct reporting to the Site Medical Director and Site Operations Director.


KEY RESPONSIBILITIES:

  • Develops and implements effective patient care strategies.
  • Ensures continuity of care for patients.
  • Provides holistic care, including referrals to additional healthcare specialists.
  • Maintains thorough, clear, and accurate documentation of patient interactions.
  • Conducts and records appropriate periodic health evaluations.
  • Offers relevant patient education.
  • Exhibits professional work habits, including punctuality, timely communication, adherence to universal precautions, accurate documentation, patient confidentiality, and proper scheduling protocols.
  • Completes a minimum of 2946 outpatient consultations with patients annually.
  • Engages in designated Quality Assurance committees.
  • Regularly participates in Team/Clinical meetings.
  • Takes part in nurse practitioner performance evaluations as assigned.
  • Contributes to the development and upkeep of practice guidelines as needed.
  • Participates in assigned QA Committee activities.
  • Attends mandatory in-service training sessions.
  • Represents the organization in community presentations when requested.
  • Acts as a representative of the provider staff at local boards or organizations as needed.
  • Provides instruction to students as assigned.
  • Completes annual continuing education requirements for certification.
  • Attends regional or national meetings related to Community Health Centers as required.
  • All staff members are dedicated to participating in quality improvement initiatives that align with Patient-Centered Medical Home (PCMH) principles.
  • Performs additional duties as assigned.
ESSENTIAL QUALIFICATIONS:
Education/Training

  • Graduate from an accredited nursing and nurse practitioner program.
  • Master’s degree in Nursing.
  • Valid registered nurse license in Indiana.
  • Certification as a nurse practitioner from an accredited certifying body.
  • Prescriptive authority and DEA certification.
  • Current CPR certification.
Experience

  • Minimum of 1-3 years of experience as a Nurse Practitioner or significant experience as a Registered Nurse in emergency or intensive care settings.
Skills/Job Requirements

  • Strong organizational and time management skills.
  • Proven capability to collaborate effectively within a team.
  • Excellent written and verbal communication skills.
  • Ability to remain adaptable and flexible.
  • Bilingual in Spanish is preferred.
  • Valid driver’s license and reliable transportation.
  • Capacity to adhere to organizational policies and procedures.
Technology Skills

  • Proficient in operating multi-line phone systems and office equipment, including printers and fax machines.
  • Basic proficiency in software applications (Microsoft Office, EHR, online resources, etc.).

  • Completion of all assigned Relias training.
