Nurse Practitioner Resident


As a Nurse Practitioner, you will deliver comprehensive health care services to individuals and families, focusing on health promotion and disease prevention, while facilitating referrals to physicians and other members of the healthcare team as necessary.

This role involves close collaboration with clinical personnel and will report to the Site Medical Director and Site Operations Director.


KEY RESPONSIBILITIES:

  • Develops and implements effective patient care strategies.
  • Ensures continuity of care for patients.
  • Provides holistic care, including referrals to additional healthcare professionals.
  • Maintains clear, concise, and accurate documentation of patient interactions.
  • Conducts and records appropriate periodic health evaluations.
  • Delivers relevant patient education.
  • Exhibits professional work habits, including punctuality, timely communication, adherence to universal precautions, accurate documentation, patient confidentiality, and proper scheduling protocols.
  • Completes a minimum of 2946 outpatient consultations with patients annually.
  • Engages in assigned Quality Assurance initiatives.
  • Regularly participates in Team/Clinical meetings.
  • Contributes to nurse practitioner performance evaluations as assigned.
  • Assists in the formulation and upkeep of practice guidelines as designated.
  • Participates in the QA Committee as required.
  • Attends mandatory in-service training sessions.
  • Delivers community presentations when requested.
  • Represents the provider team at local organizations or boards if needed.
  • Instructs students as assigned.
  • Completes annual continuing education requirements for certification.
  • Attends regional or national meetings related to Community Health Centers as needed.
  • All HealthLinc staff are dedicated to engaging in quality improvement efforts that align with and support Patient-Centered Medical Home (PCMH) principles.
  • Performs additional duties as assigned.
QUALIFICATIONS:

EDUCATION/TRAINING:
  • Graduate of an accredited nursing and nurse practitioner program.
  • Master's degree in Nursing.
  • Current registered nurse license in Indiana.
  • Certification as a nurse practitioner from an accredited certifying body.
  • Prescriptive authority and DEA certification.
  • Current CPR certification.
EXPERIENCE:
  • 1-3 years of experience as a Nurse Practitioner or significant experience as a Registered Nurse in emergency or intensive care settings.
SKILLS:
  • Exceptional customer service abilities.
  • Strong organizational and time management skills.
  • Proven capacity to collaborate effectively within a team.
  • Excellent written and verbal communication skills.
  • Ability to adapt and remain flexible.
  • Bilingual in Spanish is preferred.
  • Possession of a valid driver's license and reliable transportation.
  • Ability to adhere to HealthLinc policies and procedures.
TECHNOLOGY SKILLS:
  • Proficient in operating multi-line phone systems and office equipment such as printers and fax machines.
  • Basic proficiency in software applications (Microsoft Office, EHR, online resources, etc.).

  • N/A

TRAINING:
  • Completion of all assigned Relias training.
