Leadership Training Program Case Manager

At Fresh Lifelines for Youth, we are seeking a highly motivated and dedicated individual to join our team as a Leadership Training Program Case Manager. This is a unique opportunity to work with a dynamic organization that is committed to empowering youth to become changemakers in their communities.

Key Responsibilities:

• Work with the Leadership team to identify and recruit youth for the Leadership program
• Develop and implement a comprehensive case management plan for each youth on your caseload
• Provide ongoing support and advocacy for youth, including attending court hearings, probation meetings, and school meetings
• Connect youth to community resources and services to address their needs and goals
• Collaborate with the Leadership team to plan and facilitate pro-social activities, including events, team-building exercises, and community service projects

Requirements:

• A deep commitment to equity, social justice, and youth empowerment
• Strong relationship-building skills and ability to work with diverse populations
• Excellent communication and problem-solving skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
