We are seeking a highly skilled and compassionate Clinical Case Manager to join our team at Fred Finch Youth & Family Services. As a Clinical Case Manager, you will play a critical role in providing high-quality, trauma-informed therapeutic services and case management to at-risk youth, young adults, and families.

Key Responsibilities:
  • Provide individualized case management services to clients, including assessment, goal setting, and service planning.
  • Develop and implement trauma-informed treatment plans that address the unique needs of each client.
  • Collaborate with community partners, including schools, healthcare providers, and social services agencies, to ensure comprehensive support for clients.
  • Conduct home visits, school visits, and community-based interventions as needed.
  • Document client progress and maintain accurate records.
Requirements:
  • M.A. Degree in Social Work or Counseling from an accredited college or university.
  • Registered intern or associate with the Board of Behavioral Sciences.
  • Experience working with at-risk youth, young adults, and families.
  • Strong knowledge of trauma-informed care and evidence-based practices.
  • Excellent communication and interpersonal skills.
