Job Title: Case Manager

At Fresh Lifelines for Youth, we are seeking a highly motivated and dedicated Case Manager to join our team. The ideal candidate will have a deep commitment to equity, social, racial, and economic justice.

Job Summary:

The Case Manager will be responsible for developing a group of approximately 13 youth from a Leadership Cohort of 26. This includes connecting them to resources in the community, attending family, probation, school, and other external program meetings, and working with youth 1:1 and in small groups to set goals around leadership, education, and building skills.

Key Responsibilities:
  1. Recruit and engage youth in the Leadership program, working with the Leadership team and FLY's Law team to identify potential participants.
  2. Build relationships with each youth on your caseload, meeting 1:1 monthly to develop SMART goals and identify needs and action steps.
  3. Provide ongoing case/crisis management, utilizing a healing-informed and strengths-based approach to respond to youth crisis.
  4. Maintain thorough case management paperwork, including assessments, case management notes, client contracts, and development plans.
  5. Plan and facilitate activities for the Leadership cohort, including fun events, team-building exercises, and community service projects.
  6. Engage youth in program opportunities to deepen historical and cultural understanding of their experiences and community issues.
  7. Attend and actively participate in community meetings, Multi-Service Team (MST) aligning with program and youth needs and opportunities.
Requirements:

The ideal candidate will have strong relationship-building skills, excellent note-taking ability, and a track record of being highly organized.
