Compensation: Hourly rate plus travel reimbursement

Role Summary: We are in search of a Community Outreach Partner to lead our community engagement efforts in Baltimore. The ideal candidate will be a local advocate with a strong connection to community needs and a passion for promoting health equity and social justice. This role involves close collaboration to create and execute effective outreach strategies that prioritize community interests.

Key Responsibilities:

  • Act as a bridge between CareAdvisors and local communities, ensuring their concerns are acknowledged and addressed.
  • Conduct home visits within assigned communities, offering tailored support and assistance.
  • Lead outreach initiatives at community gatherings, sharing information about available resources and facilitating enrollments.
  • Provide timely assistance and resources to community members, considering their cultural and socio-economic contexts.
  • Deliver post-event support, including follow-ups with participants to ensure ongoing care and assistance.
  • Accurately complete necessary documentation and data entry in a timely manner.
  • Achieve targets set by management and actively engage in team meetings and discussions.
  • Perform additional tasks as assigned by management.

Essential Qualifications:

  • Willingness to travel 1-2 hours for visits (travel time compensated).
  • Strong commitment to community engagement and health equity advocacy.
  • Bachelor's degree or equivalent professional experience.
  • 4-6 years of experience in community outreach or a related field.
  • Experience collaborating with Community Based Organizations (CBOs).
  • Flexible availability, including potential weekend work.
  • Access to a vehicle, valid driver's license, and proof of insurance.
  • Basic computer skills and a willingness to learn new technologies.
  • Strong organizational and time management abilities.
  • Excellent verbal and written communication skills.
  • Bilingual skills are preferred.
  • Ability to pass pre-employment background checks and drug screenings.
  • Capacity to work independently while managing time effectively.
  • Commitment to a minimum of 3 months of service.
