Key Responsibilities:

  • Community Outreach Worker I:
    • Identify and recruit potential clients within the community through effective outreach strategies.
    • Provide essential information regarding program objectives, services, and eligibility criteria to individuals and families.
    • Conduct assessments of clients' living conditions to ensure a safe and healthy environment.
    • Act as a liaison between clients and community resources, including various social services agencies.
    • Coordinate appointments for clients with healthcare providers and offer support during these visits.
    • Maintain client engagement through follow-ups and progress assessments.
    • Assist in the organization and execution of community events such as health fairs and workshops.
    • Participate in team meetings to discuss outreach activities and address any concerns.
    • Document outreach efforts and maintain accurate records.
    • Foster positive relationships with community members.
  • Community Outreach Worker II:
    • Provide crisis intervention and individual counseling to clients, along with basic health education.
    • Conduct initial assessments to identify individual client needs.
    • Engage with neighborhood groups and attend community meetings to promote services and address community issues.
    • Evaluate educational materials and methods to meet the needs of the client community.
    • May offer guidance and training to other outreach workers.
    • Perform all duties associated with the Community Outreach Worker I role.

Qualifications:

  • Community Outreach Worker I: High school diploma or GED; no prior experience required.
  • Community Outreach Worker II: High school diploma or GED; two years of experience in community outreach, healthcare, research, or social services.

Skills Required: Strong communication skills, ability to establish effective working relationships, proficiency in office equipment, report writing, and maintaining confidentiality. Candidates must be able to work in various weather conditions and handle sensitive matters with discretion.

Conditions of Employment: Candidates may be required to use their personal vehicle for official duties and possess a valid driver's license. A background check may be conducted as part of the hiring process.

Compensation: Salary ranges for the Community Outreach Worker I and II positions are competitive and commensurate with education and experience.
