The Licensed Practical Nurse (LPN) plays a crucial role within the healthcare team, delivering high-quality nursing care in accordance with North Oaks Health System's established standards.

This position involves providing direct nursing services to patients across various age groups, including adults, children, and the elderly, while adapting care methods to meet the unique needs of each demographic.

The LPN is expected to embody the Philosophy and Objectives of North Oaks Health System, fostering effective communication among nursing staff, physicians, other departments, patients, and their families.

Participation in committees may be required as directed by the Clinic Director. The LPN's responsibilities are confined to the guidelines set forth by the Louisiana State Board of Nursing.

Qualifications:

Education:

Required Education:
Graduation from an accredited nursing program is essential. Membership in a professional LPN organization is encouraged.

Licensure/Certification:
Current licensure by the Louisiana State Board of Practical Nurse Examiners is mandatory, with ongoing maintenance of licensure.

Experience:
A minimum of 1-3 years of prior LPN experience is preferred, particularly in a clinical environment.

Key Responsibilities:

  • Maintain high standards of patient care.
a. Greet patients with courtesy and professionalism.

b. Call patients from the waiting area based on chart availability.

Document and record patient metrics such as weight, height, and vital signs, escorting them to the examination room and preparing them for their physician visit.



c. Accurately document current medications and chief complaints in the patient's medical record. Responsible for conducting pain assessments and maintaining the Master Problem List. Reassess and document as necessary. Provide patient education and maintain documentation.

d. Assist physicians during examinations and procedures, including preparation.

e. Administer medications safely and effectively, adhering to scope of practice and policy.

f. Perform phlebotomy and other ancillary duties as required by the physician and according to policy. Conduct necessary drug and alcohol testing as per departmental guidelines.

g.

Assist in the processing of employee medical information and test results, ensuring accuracy and confidentiality.

Support clients in obtaining required medical information.

h. Aid in the application of sterile dressings.

i. Help with the cleaning and returning of instruments to the Sterile Services Department for sterilization.

Assist in the collection and preparation of patient specimens for transport, following Universal Precaution Policy and laboratory requirements.

k. Ensure cleanliness of the patient treatment area post-visit.

l. Conduct waived laboratory testing as ordered. (Refer to specific laboratory policy and procedure manual.) m. Perform daily quality control and checklist functions as assigned. (Refer to specific policy and procedure manual.)

m. Attend to the psychosocial needs of patients and their significant others through appropriate interaction.

n. Provide assistance to physicians and staff as needed.

o. Process medication refills and respond to patient messages as directed by a physician.

p. Issue physician's excuses for school or work, documenting appropriately in the patient's chart.

q. Schedule hospital tests as directed by a physician, ensuring proper pre-authorization and medical justification.

r. Communicate test results to patients as directed by a physician.

s. Assist in the retrieval, logging, and stocking of sample medications.

t. Maintain accurate and legible documentation at all times. Follow up with patients regarding diagnostic testing results and document all communications appropriately. Ensure proper documentation of verbal/telephone orders from physicians. Complete all necessary forms, including those for workers' compensation and laboratory references.

a. Follow North Oaks Health System's Compliance Program and relevant regulatory guidelines.

b. Attend monthly staff meetings consistently.

c. Maintain an acceptable dress code.

d. Report all incidents and injuries to the appropriate supervisor.

e. Report any medication errors.

f. Comply with attendance policies.

g. Be knowledgeable of hospital safety codes.

h. Report unsafe conditions promptly.

i. Ensure a clean working environment.

j. Account for medications, conducting counts as required and labeling multi-dose medications appropriately.

  • Assist in operational functions.
a. Support clinic coordinators and the Director in maintaining policies and procedures as needed.

b. Help maintain inventory, checking for expired items monthly and notifying supervisors when stock is low. Clean and stock examination rooms as necessary.

  • Maintain effective communication.
a. Communicate clearly and listen actively, ensuring complete follow-up on matters requiring attention.

b. Foster positive relationships with patients, visitors, staff, and physicians through courteous and professional communication.

c. Coordinate referrals, hospital testing, and admissions with the appropriate personnel. Assist with office duties, including answering calls, pulling charts, filing, and audits.
