Job Summary

We are seeking a highly skilled and compassionate Pediatric Private Duty LPN (Licensed Practical Nurse) to join our team in the Hammond area. As a Pediatric Private Duty LPN, you will provide high-quality, patient-centered care to pediatric patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Provide Direct Nursing Care: As a Pediatric Private Duty LPN, you will provide direct nursing care to pediatric patients in their homes, following the plan of care and in coordination with other members of the healthcare team.
  • Document Patient Visits: You will thoroughly and completely document patient visits in the electronic medical record, following regulatory and payer requirements.
  • Participate in Care Coordination: You will actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
  • Ensure Confidentiality: You will ensure the confidentiality of patient health information, following agency policy.
Requirements
  • Current LPN Licensure: You must have current LPN licensure in the state of practice.
  • Experience: Previous home care experience is preferred.
  • CPR Certification: You must have current CPR certification.
