Job Overview

The Phoenix at Hammond is seeking a dedicated and compassionate Wellness Nurse to join their team.

The LPN (Nurse) reports directly to the Wellness Director and is responsible for monitoring the health and wellbeing of our residents.

Key Responsibilities:

  • Monitor and assess the health and wellbeing of residents
  • Provide leadership support and assistance with resident care and services
  • Administer medication and maintain clinical quality assurance
  • Communicate with resident support services and families
  • Lead and demonstrate the elements of wellness that are core components of Phoenix programs and services

Principle Duties and Responsibilities:

  • Resident Care
  • Identify ongoing needs and services of residents through the assessment and Personalized Service Plan (PSP) process
  • Ensure proper follow through and documentation for residents with a change in clinical care needs
  • Assist the Wellness Director with completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident's monthly status
  • Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out
  • Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates
  • Assist with direct resident care as needed

Requirements:

  • Must be a Licensed Practical Nurse or Registered Nurse
  • A minimum of one year of experience working with elders or disabled individuals, preferred
  • Must have demonstrated leadership capabilities
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
  • Ability to work weekends, evenings, and flexible hours, available to our customers at peak service delivery days and times
