Job Summary:

We are seeking a highly organized and self-motivated Patient Care Coordinator to join our team. The ideal candidate will have above-average computer skills and be able to facilitate correspondence between patients, their family, medical and administrative staff, and healthcare providers. This role requires administrative duties and excellent customer service skills.

Responsibilities & Duties:

* Greet patients upon arrival and answer phone calls and address concerns and questions
* Schedule patients for appointments and gather necessary information
* Make copies of insurance cards, IDs, medication lists, and referrals
* Provide new patient paperwork and explain HIPAA-related forms
* Field and triage patient questions and make confirmation phone calls for appointments
* Follow up on cancellations, reschedules, and no-shows
* Check out patients, collect payments, and schedule follow-up appointments
* Update and scan patient chart notes and document encounters
* Process and follow up on referrals
* Insurance verification
* Minor office cleaning and upkeep

Qualifications & Skills:

* High school diploma or GED
* Organized and efficient
* Excellent customer service skills
* Ability to multitask and maintain a positive attitude
* Self-motivated and a team player
