  • Conducting preliminary health assessments for newly enrolled patients
  • Coordinating community resources and services to meet patient needs
  • Providing education and support on chronic condition management

We offer a competitive salary ranging from $21/hr to $28/hr, as well as a comprehensive benefits package including health insurance, 401k, and paid time off.
