Job Description:The LVN will work closely with patients, families, and healthcare teams to provide compassionate and individualized care. - Process patients in and out, ensuring accurate documentation and timely interventions.
- Delegate nursing rounds and administer medications as prescribed by physicians.
- Maintain accurate and up-to-date patient records, adhering to regulatory requirements.
To be considered for this role, applicants must possess a current and active license to practice as an LVN in this state. A minimum of one year of experience as a nursing assistant in a skilled rehabilitation or long-term care environment is preferred. Electronic medical records experience is also beneficial.
