Job Description:

The LVN will work closely with patients, families, and healthcare teams to provide compassionate and individualized care. This role requires strong communication skills, attention to detail, and the ability to prioritize tasks effectively.   • Process patients in and out, ensuring accurate documentation and timely interventions.
  • Delegate nursing rounds and administer medications as prescribed by physicians.
  • Maintain accurate and up-to-date patient records, adhering to regulatory requirements.
Requirements:

To be considered for this role, applicants must possess a current and active license to practice as an LVN in this state. A minimum of one year of experience as a nursing assistant in a skilled rehabilitation or long-term care environment is preferred. Electronic medical records experience is also beneficial.
