Hospice Care Professional

HopeHealth Hospice & Palliative Care seeks a skilled Clinical Social Worker to join its team. Located in Rhode Island, this position offers a rewarding opportunity to make a difference in patients' lives.
Job OverviewThis full-time 40 hour per week position is responsible for planning, implementing, and evaluating social work services for patients in their homes or long-term care facilities. The ideal candidate will have experience working with family systems, hospice care, and medical social work.
Responsibilities- Conduct comprehensive psychosocial assessments to identify patients' needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
- Provide accurate and timely documentation of patient interactions and progress in the electronic medical record.
- Travel to patients' homes within the designated service area to provide in-person support and care.
- Licensed as an LSW/LCSW/LICSW
- Relevant clinical experience in healthcare, preferably in medical social work or hospice care
- Familiarity with family systems and group dynamics
- A competitive salary: $ $70,000 - $90,000 per year, depending on experience
