Job Overview

We invite you to join North Homes Inc. as a Client Care Specialist, where you will be instrumental in delivering exceptional support to clients and their families. Your role will involve facilitating connections to personalized services that cater to individual needs. By managing referrals and coordinating both internal and external resources, you will ensure that clients receive the comprehensive assistance necessary for their well-being.

Key Responsibilities:

  • Conducting assessments to identify client needs during the intake and initial evaluation process.
  • Linking clients and their families with appropriate services and resources.
  • Ensuring seamless continuity of care among the client's treatment team.
  • Promoting collaboration between internal and external stakeholders to create integrated, person-centered treatment plans.
  • Evaluating and supporting clients facing Social Determinants of Health (SDOH).
  • Working closely with referral resources both internally and externally.
  • Participating in a rotation for handling new inquiries and referrals.
  • Assisting clients with the annual update of intake documentation and release of information.
  • Documenting and maintaining precise and timely summaries of care coordination.
  • Supporting assigned providers with client scheduling and record requests for care coordination.
  • Tracking and overseeing referral processes.
  • Attending multidisciplinary team meetings, recording notes, and documenting attendance.
  • Monitoring the Encounter Alert Service (EAS) and following up with clients within 24 hours post-discharge from emergency or hospital care.
  • Collecting and entering necessary data into the client's Electronic Care Record (ECR).
  • Monitoring the performance of providers assigned to caseloads.
  • Performing additional duties as assigned by the supervisor.

Qualifications:

To qualify for the position of Client Care Specialist, candidates must meet the following:

Minimum Requirements:

  • High School Diploma or GED.
  • Non-judgmental attitude towards diverse lifestyles, races, and economic backgrounds.
  • Ability to pass a background study and comply with relevant regulations.
  • Must be at least 21 years of age and possess a valid driver's license.

Preferred Experience:

  • Knowledge and experience in behavioral health services.
  • Two years of college education in a healthcare or human services field, or three years of relevant experience in a behavioral health clinic or social services setting.

Physical Requirements:

  • Ability to sit for extended periods while working on a computer.
  • Capability to lift up to 15 pounds occasionally.
