Care Coordinator Specialist

Job Summary:
North Homes Inc. is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in connecting clients and their families with tailored services, based on their unique needs.
Key Responsibilities:
- Assess Client Needs: Conduct thorough intake and preliminary screening processes to identify client needs and develop personalized care plans.
- Connect Clients with Services: Facilitate connections with internal and external resources to ensure clients receive the support they need to thrive.
- Ensure Continuity of Care: Coordinate with treatment teams to ensure seamless transitions and continuity of care.
- Develop Integrated Treatment Plans: Collaborate with internal and external providers to develop person-centered and family-centered integrated treatment plans.
- Assess and Assist with Social Determinants of Health: Identify and address Social Determinants of Health (SDOH) to promote overall well-being.
- Collaborate with Referral Resources: Work closely with internal and external referral resources to ensure timely and effective connections.
- Manage Intake and Referral Processes: Rotate intake weeks for new inquiries and referrals, and assist clients with updating intake paperwork and ROI on an annual basis.
- Document and Maintain Accurate Records: Document and maintain accurate and timely care coordination summaries, and ensure compliance with regulatory requirements.
- Monitor and Track Referrals: Track and monitor referrals, and follow up with clients within 24 hours of discharge from the ED or hospital.
- Collect and Enter Required Data: Collect and enter required data into client's Electronic Client Record (ECR).
- Monitor and Support Providers: Monitor providers assigned to caseloads, and assist with scheduling clients, requesting and disclosing records for care coordination.
- Attend Multidisciplinary Team Meetings: Attend integrated multidisciplinary team meetings, and document meeting notes and attendance.
Requirements:
- Education: High School Graduate or GED
- Background Study: Must be able to pass a DHS background study and meet applicable requirements and regulations.
- Age and License: Must be at least 21 years old and have a valid Minnesota Driver's License.
Preferred Experience:
- Behavioral Health Experience: Experience and knowledge in behavioral health programming.
- Education and Experience: 2 years of college in a healthcare or human service related field, or 3 years of experience preferably working within a behavioral health clinic or social services.
Skills and Abilities:
- Client-Centered Care: Excellent client-centered care, interpersonal, and organizational skills.
- Communication: Excellent oral and written communication skills.
- Teamwork: Ability to be a team player and work collaboratively with internal and external providers and agencies.
- Adaptability: Ability to adapt and deal with challenging situations and changes.
- Electronic Medical Record (EMR) System: Ability to use Electronic Medical Record (EMR) system.
- Confidentiality: Maintain confidentiality and handle sensitive information with discretion.
Physical Requirements:
- Prolonged Sitting: Prolonged periods of sitting at a desk and working on a computer.
- Lifting: Must be able to lift up to 15 pounds at times.
