**Job Description:**

We are seeking an experienced Medical Care Navigator Lead to join our team at Community of Hope in Washington, DC.

The Medical Care Navigator Lead will be responsible for leading a team of Nurse Navigators and managing the day-to-day effectiveness of the care management service delivery model. This includes evaluating program outcomes, individual staff productivity, and performance. The successful candidate will also deliver comprehensive care-management services, including transition of care planning, patient education, and social service referrals and linkages, in a timely and culturally appropriate manner through a patient-centered team-based care mode.

**Responsibilities:**

  • Develops clinical elements of individual care plans, including biopsychosocial assessments of patient needs, in consultation with other health team members.
  • Ensures that care plans align with provider orders and COH standards of care for chronic diseases.
  • Monitors patient health status and documents progress toward goals contained in person-centered plan of care, amending as needed.
  • Implements person-centered plan of care through linkages, referrals, and coordination with necessary services and supports.
  • Works with healthcare team members within and outside COH to ensure continuity of care and reduce fragmentation, duplication, and gaps in treatment.
  • Liaises between patients and caregivers, providers, clinical staff, specialists, and other healthcare professionals.
  • Facilitates patient empowerment and quality of life by promoting educated, independent patient choice on all aspects of care.
  • Provides education to patients and caregivers to understand health condition, medications, and self-care skills.
  • Counsels patients on utilizing health services to avoid unnecessary emergency department and hospital utilization.
  • Coordinates transitions between healthcare providers and settings to reduce emergency department and inpatient admissions, readmissions, and length of stay.

**Requirements:**

  • Bachelor of science degree in nursing required.
  • 1-2 years of supervisory experience preferred.
  • Current, unencumbered DC Registered Nurse license with current CPR certification required.
  • Knowledge of primary care and health maintenance required.
  • Experience educating patients with chronic health conditions preferred.
  • Ability to work with computers and electronic health records required.
  • Strong verbal and written communication skills required.
  • Strong organizational skills with ability to multitask required.
  • Demonstrated cultural competence in communicating with low-income populations required.
  • Demonstrated ability to function effectively in a team required.
  • Ability to travel between COH sites or relocate to different COH site on full or part-time basis required.
  • Ability to work Saturdays and evenings required.

**Benefits:**

  • Competitive salary range $100,000 - $110,000 annually.
  • 8-hour workdays with paid lunch.
