Job Summary

We are seeking an experienced Oncology Care Navigator to join our team at Prisma Health. This is a rewarding opportunity to make a positive impact on the lives of patients and their families.

In this role, you will be responsible for coordinating care for oncology patients across the continuum of care. This includes evaluating new patient cancer referrals, reviewing patient records, assessing health status, and ensuring patient referrals to appropriate support services.

You will also participate in the development of assessment tools, care management guidelines, and pathways for cross-continuum care of patients with cancer. Additionally, you will conduct patient education, collaborate with patients to establish goals, and promote patient self-management.

Key Responsibilities:

  • Evaluate new patient cancer referrals and initiate contact with patients
  • Review patient records for NCCN guidelines and identify staging studies and molecular profiling
  • Assess health status of patients targeted for participation in cancer care management programs
  • Ensure patient referrals to appropriate support services
  • Participate in the development of assessment tools and care management guidelines
  • Conduct patient education and promote patient self-management

Requirements:

  • Bachelor's Degree - Nursing
  • 3 years - Oncology experience
  • Licensed to practice as a Registered Nurse in South Carolina
  • Oncology certification (OCN or ONN-CG)
