Position Overview

Become an integral part of a leading academic healthcare institution, The University of Chicago Medicine, as a Clinical Pharmacist Specialist within the Outpatient Primary Care Clinic. In this role, you will be a licensed pharmacist responsible for ensuring the delivery of safe, effective, and timely medication therapy. Your focus will be on providing both clinical and distributive pharmacy services to patients in a designated area.

Our ambulatory pharmacists are dedicated to offering direct patient care to adult populations. This position is tailored to support one of our most dynamic clinics, Primary Care, where you will collaborate with esteemed healthcare providers. The role involves weekday operations in the Primary Care clinic, participation in the Med2Beds discharge program every six weeks, and on-call responsibilities once a quarter. The UCM Ambulatory Clinical Pharmacy group is committed to maximizing patient outcomes through effective medication management.

Key Responsibilities

  • Lead designated patient service lines, clinical areas, and therapeutic initiatives.
  • Conduct in-clinic visits, follow-up monitoring, patient education, medication reconciliation, and provide vaccine recommendations.
  • Facilitate transitions of care between inpatient and outpatient services.
  • Coordinate access and affordability for advanced biologic therapies prior to initiation.
  • Deliver direct patient care and engage in clinical practice across various programs.
  • Support pharmacy operations and medication dispensing processes.
  • Participate in experiential education and advancement of pharmacy practice.
  • Perform additional duties as required.

Qualifications

  • Graduation from an ACPE accredited School of Pharmacy.
  • Comprehensive knowledge of contemporary hospital and clinical practices.
  • Understanding of hospital operations and regulatory compliance.
  • Excellent written and verbal communication skills.
  • Ability to collaborate effectively with supervisors, colleagues, and support staff.
  • Capability to integrate clinical and distributive pharmacy services with quality improvement and professional development.
  • Extensive clinical knowledge and expertise in patient-centered care.
  • Strong teaching and precepting skills, with experience in experiential training.
  • Eligibility for licensure as a pharmacist in Illinois or completion of the licensure process within a specified timeframe.
  • Board Certification in the relevant area is required within a designated period post-hire.

Preferred Qualifications

  • Doctor of Pharmacy or an equivalent advanced degree.
  • Completion of an ASHP accredited PGY-1 Pharmacy residency.
  • Clinical pharmacy experience in a hospital setting (2-4 years preferred).
  • Completion of an ASHP accredited PGY-2 Pharmacy residency.

In this position, you will typically work around 40 hours per week during the day shift, with a schedule primarily from Monday to Friday, and every sixth Saturday.
