Position Overview

Become an integral part of a prestigious academic healthcare institution, The University of Chicago Medicine, as a Clinical Pharmacist Specialist in the Outpatient Primary Care Clinic. In this role, you will be a licensed pharmacist responsible for ensuring the delivery of safe, effective, and timely medication therapy. Your focus will be on providing both clinical and distributive pharmacy services to patients within a designated area.

As an ambulatory pharmacist, you will engage in direct patient care for adult populations, supporting the pharmacy operations and clinical requirements across multiple outpatient clinics. This specific position is tailored to bolster one of our most dynamic clinics, Primary Care, collaborating with esteemed healthcare providers. Your work schedule will primarily be weekdays, with participation in the Med2Beds discharge program on a rotational Saturday basis and on-call duties every quarter.

Key Responsibilities

  • Lead designated patient service lines and therapeutic programs.
  • Conduct in-clinic visits, follow-up monitoring, patient education, medication reconciliation, and respond to drug information inquiries from healthcare providers and multidisciplinary teams.
  • Facilitate transitions of care between inpatient and outpatient services.
  • Coordinate access to advanced biologics, ensuring affordability prior to initiation.
  • Deliver direct patient care and engage in clinical practice across various service-based programs.
  • Participate in pharmacy operations and medication dispensing activities.
  • Contribute to experiential education and practice advancement initiatives.
  • Perform additional duties as required.

Qualifications

  • Graduation from an ACPE accredited School of Pharmacy.
  • Familiarity with contemporary hospital and clinical practices.
  • Understanding of hospital operations and regulatory compliance.
  • Excellent written and verbal communication skills.
  • Ability to collaborate effectively with colleagues and support staff.
  • Capability to integrate clinical and distributive pharmacy services with quality improvement and professional development.
  • Extensive clinical knowledge in traditional therapeutics and patient-centered care.
  • Expertise in advanced pharmacy practice and specialty medication management.
  • Strong teaching and precepting skills.
  • Eligibility for licensure as a pharmacist in Illinois or completion of the licensure process within a specified timeframe.
  • Board Certification in a relevant area required within a designated period after hiring.

Preferred Qualifications

  • Doctor of Pharmacy or equivalent advanced degree.
  • Completion of an ASHP accredited PGY-1 Pharmacy residency.
  • Clinical pharmacy experience in a hospital environment.
  • Completion of an ASHP accredited PGY-2 Pharmacy residency.

Work Environment

  • Full-Time Position.
  • Day Shift Schedule.
  • Located in the Hyde Park area.
  • Non-Union Position.

In this role, you will typically work around 40 hours per week, primarily during the day shift. The anticipated schedule will be Monday through Friday, with occasional Saturday responsibilities.
