As an ambulatory care pharmacist, you will engage in direct patient care for adult populations, supporting the pharmacy operations and clinical requirements across multiple outpatient clinics. This position is tailored to enhance the services of one of our prominent clinics, focusing on Primary Care, where you will collaborate with esteemed healthcare providers. The role includes weekday responsibilities in the Primary Care clinic, participation in the Med2Beds discharge program on a rotating Saturday schedule, and on-call duties quarterly, subject to adjustments. The UCM Ambulatory Clinical Pharmacy team is committed to maximizing patient outcomes through effective medication management.
Key Responsibilities
- Lead specific patient service lines, clinical areas, and therapeutic initiatives.
- Conduct in-clinic visits, follow-up assessments, patient education, medication reconciliation, and lab monitoring.
- Facilitate transitions of care between inpatient and outpatient services.
- Coordinate access to advanced biologic therapies, ensuring affordability prior to initiation.
- Deliver direct patient care and engage in clinical practice across decentralized and service-based programs.
- Provide pharmaceutical services throughout the medical center.
- Participate in pharmacy operations and medication dispensing activities.
- Support experiential education and advancement of pharmacy practice.
- Perform additional duties as required.
Qualifications
- Graduation from an ACPE accredited School of Pharmacy.
- Familiarity with contemporary hospital and clinical practices.
- Understanding of hospital operations and regulatory compliance.
- Excellent written and verbal communication skills.
- Ability to collaborate effectively with colleagues and support staff.
- Capacity to integrate clinical and distributive pharmacy services with quality improvement and professional development.
- Extensive clinical knowledge in traditional therapeutics and patient-centered care.
- Expertise in advanced pharmacy practice and specialty medication management.
- Strong teaching and mentoring capabilities.
- Eligibility for pharmacist licensure in Illinois or completion of the licensure process within 60 days of employment.
- Board Certification in the relevant area is required within 18 months of hire.
Preferred Qualifications
- Doctor of Pharmacy or another advanced degree.
- Completion of an ASHP accredited PGY-1 Pharmacy residency.
- Clinical pharmacy experience (2-4 years) in a hospital environment.
- Completion of an ASHP accredited PGY-2 Pharmacy residency.

In this position, you will typically work around 40 hours per week, primarily during the day shift, with a schedule of Monday through Friday and one Saturday every six weeks.
