POSITION OVERVIEW: The Critical Care Pharmacy Specialist is responsible for delivering exceptional pharmaceutical care, ensuring the safe and effective use of medications for all patients. This role involves close collaboration with healthcare professionals and patients to enhance medical care and improve quality of life, with a strong focus on monitoring and documenting pharmacotherapy outcomes. The specialist will also provide comprehensive drug information to medical staff and engage in various operational and clinical projects as assigned.

KEY RESPONSIBILITIES:

  • Engage in daily patient care discussions, assessing medication therapies for appropriateness, dosage, and potential interactions. Communicate necessary clarifications with prescribers and document all modifications in patient records.
  • Regularly update patient profiles, including demographics and medication histories, while promoting formulary compliance and managing non-formulary drug procurement as needed.
  • Demonstrate proficiency in managing the pharmaceutical needs of diverse patient populations, including adults, pediatrics, geriatrics, and those with psychiatric conditions.
  • Review pharmacy records to ensure the safety and appropriateness of drug therapies, identifying and documenting any adverse reactions or medication errors.
  • Conduct pharmacokinetic assessments, facilitate IV to PO conversions, and perform other clinical pharmacy functions efficiently and accurately.
  • Advocate for safe and effective medication use by participating in clinical pharmacy services and medication utilization initiatives, documenting interventions and cost savings.
  • Act as a consultant for the Pharmacy & Therapeutics committee and contribute to clinical performance improvement initiatives.
  • Participate in clinical rounds, public speaking engagements, and the development of educational materials for pharmacy staff and healthcare professionals.
  • Collaborate in the planning and execution of system-wide pharmaceutical care programs aimed at optimizing medication therapy outcomes.
  • Ensure compliance with all regulatory standards and provide comprehensive patient care that meets established guidelines.
  • Report regularly to the Clinical Pharmacy Manager regarding operational activities and project progress.
  • Respond to medical emergencies as necessary.
  • Provide patient counseling on discharge medications, including education for ICU and behavioral health patients, and coordinate medication management with outpatient services.
  • Work rotating shifts as required.
QUALIFICATIONS:
Education:
  • Pharm.D. or equivalent from an accredited School of Pharmacy, with a current pharmacist license in good standing. Completion of a Critical Care PGY1 or PGY2 residency is preferred, or a minimum of three years of relevant clinical experience.
Skills:
  • Comprehensive understanding of pharmacy laws and regulations. Proficient in the application of new medications and technologies. Strong verbal and written communication skills are essential.
Abilities:
  • Capacity to work independently and manage stress effectively. Ability to collaborate with colleagues and adapt to changing workloads. Strong decision-making skills and the ability to prioritize tasks are critical.
