Job Overview

The department is dedicated to providing outpatient infusion services to both adult and pediatric patients.

Position Summary

Through various responsibilities within the department, Clinical Pharmacy Specialists support decentralized infusion areas and ensure optimal medication therapy for patients across a wide spectrum of health conditions. This role is centered on the outpatient management of oncology infusion and injectable patients. Key responsibilities include, but are not limited to: assisting with precertification, conducting insurance product and medical necessity evaluations, coordinating care with nursing staff, overseeing ambulatory medication access coordinators, addressing clinical inquiries regarding free drug or copay programs, managing prior authorization or claim denial processes, evaluating financial assistance options, facilitating Medicaid referrals, and overseeing transitions of care.

Pharmaceutical care services involve assessing patient needs, tailoring drug therapy based on age and disease-specific factors, adjusting treatment based on patient responses, and implementing clinical interventions to identify, mitigate, and prevent adverse medication events. Clinical Pharmacy Specialists act as resources and liaisons to other departments, hospital staff, and external organizations. They effectively manage and promote the appropriate use of medications in accordance with professional and regulatory standards. Additionally, they may engage in clinical research or initiatives aimed at enhancing medication-use processes or pharmacy practice. Clinical Pharmacy Specialists also provide education and training related to medication and practice, serving as mentors for pharmacy students and residents.

Key Responsibilities

  • Lead designated patient service lines and therapeutic programs.
  • Deliver direct patient care and clinical services, including decentralized and service-based initiatives.
  • Provide pharmaceutical services throughout the medical center.
  • Facilitate experiential education and advancement in practice.
  • Perform additional duties as required.

Required Qualifications

  • Graduation from an ACPE accredited School of Pharmacy.
  • Understanding of contemporary hospital and clinical practices.
  • Familiarity with general hospital operations and regulatory compliance.
  • Excellent written and verbal communication skills.
  • Ability to engage constructively with colleagues and support staff.
  • Capacity to integrate clinical and distributive pharmacy services with quality improvement, education, research, and professional development.
  • Extensive clinical knowledge and expertise in traditional therapeutics and patient-centered care.
  • In-depth knowledge and expertise in advanced pharmacy practice and specialty medication management.
  • Strong teaching and mentorship skills, particularly in experiential training.
  • Eligibility for licensure as a pharmacist in the state of Illinois.
  • Board Certification in the relevant area is required within 18 months of hire.

Preferred Qualifications

  • Doctor of Pharmacy or another advanced degree is preferred.
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency is required; PGY-2 Pharmacy Residency is highly preferred.
  • Clinical pharmacy experience (2-4 years) in a hospital environment is preferred.
  • Experience in oncology is preferred but not mandatory.
