Position Overview:

Become an integral part of a prestigious academic healthcare institution, The University of Chicago Medicine, as a Clinical Pharmacist Specialist in the Outpatient Primary Care Clinic. In this role, you will be a licensed pharmacist responsible for ensuring the safe, effective, and timely administration of medication therapy. You will deliver both clinical and distributive pharmacy services to patients receiving care in designated areas.

Role Responsibilities:

As an ambulatory pharmacist, you will provide direct patient care to adult populations across more than 15 outpatient clinics. This position is tailored to support one of our most dynamic clinics, Primary Care, where you will collaborate with esteemed healthcare providers. Your work schedule will primarily be weekdays, with participation in the Med2Beds discharge program one Saturday every six weeks and on-call duties one weekend each quarter. This schedule may be subject to modifications.

Key Duties Include:

  • Leading specific patient service lines and therapeutic programs.
  • Conducting in-clinic visits, follow-up monitoring, patient education, medication reconciliation, and responding to drug information inquiries from healthcare providers and multidisciplinary teams.
  • Facilitating transitions of care between inpatient and outpatient services.
  • Coordinating access to advanced biologic therapies to ensure affordability prior to initiation.
  • Delivering comprehensive patient care and clinical practice, including decentralized and service-based initiatives.
  • Engaging in pharmacy operations and medication dispensing.
  • Participating in experiential education and practice enhancement.
  • Performing additional duties as assigned.

Qualifications:

Required:

  • Graduation from an ACPE accredited School of Pharmacy.
  • Familiarity with contemporary hospital and clinical practices.
  • Understanding of hospital operations and regulatory compliance.
  • Excellent written and verbal communication skills.
  • Ability to interact positively with colleagues and support staff.
  • Capability to integrate clinical and distributive pharmacy services with quality improvement, education, research, and professional development.
  • Extensive clinical knowledge and expertise in patient-centered care.
  • Strong teaching and precepting abilities.
  • Eligibility for pharmacist licensure in Illinois or completion of the licensure process within 60 days of employment.
  • Board Certification in the relevant area is required within 18 months of hire.

Preferred:

  • Doctor of Pharmacy or another advanced degree.
  • Completion of ASHP accredited PGY-1 Pharmacy residency.
  • Clinical pharmacy experience (2-4 years) in a hospital environment.
  • Completion of ASHP accredited PGY-2 Pharmacy residency.

In this position, you will typically work around 40 hours per week during the day shift, from approximately 9 AM to 5:30 PM, without on-call shifts. Your schedule will predominantly be Monday through Friday, with one Saturday every six weeks.
