POSITION OVERVIEW: The Critical Care Pharmacy Specialist is responsible for ensuring the safe, effective, and economical use of medications for all patients. This role involves close collaboration with healthcare professionals and patients to enhance medical care and quality of life, with a strong emphasis on monitoring and documenting pharmacotherapy outcomes. The specialist will provide detailed drug information to medical staff, engage in operational and clinical tasks as required, and meet the qualifications necessary to care for patients across various age groups in the critical care setting.

KEY RESPONSIBILITIES:

  • Engage in daily patient care discussions, evaluating medication therapies for appropriateness, dosage, and potential interactions. Communicate any necessary clarifications with prescribers and document all modifications in patient records.
  • Maintain and regularly update patient profiles, including demographics, allergies, and current medications. Advocate for formulary compliance by converting non-formulary orders when feasible and managing the procurement of non-formulary medications as needed.
  • Demonstrate competence in treating patients across various age groups, including adults, pediatrics, geriatrics, adolescents, and those with psychiatric needs.
  • Review pharmacy records to ensure medication therapies are appropriate and safe, actively preventing, detecting, documenting, and reporting any adverse drug reactions or medication errors.
  • Conduct pharmacokinetic assessments, facilitate IV to PO conversions, and execute other clinical pharmacy functions accurately and promptly.
  • Promote safe and effective medication use by participating in clinical pharmacy services and documenting medication utilization initiatives.
  • Act as a consultant for the Pharmacy & Therapeutics committee and other clinical improvement committees.
  • Participate in clinical rounds, public speaking engagements, and the presentation of data using appropriate formats. Contribute to educational materials for pharmacy staff and healthcare professionals.
  • Assist in the planning and execution of system-wide pharmaceutical care programs aimed at optimizing drug therapy outcomes, completing one clinical project annually in collaboration with the pharmacy clinical team.
  • Ensure that all patient care activities comply with relevant regulations and standards.
  • Report regularly to the Clinical Pharmacy Manager regarding operations, assigned projects, and overall responsibilities.
  • Respond to medical emergencies as required.
  • Provide counseling to patients regarding discharge medications and coordinate with outpatient pharmacy services as necessary.
  • Work rotating shifts, including both AM and PM schedules.

JOB REQUIREMENTS:

Education and Experience:

  • Pharm.D. or equivalent from an ACPE-accredited School of Pharmacy, with a current pharmacist license in good standing. Critical Care residency training or a minimum of three years of relevant clinical experience is preferred. Board Certification is recommended.

Skills and Abilities:

  • Comprehensive knowledge of pharmacy laws and regulations. Proficiency in utilizing new medications and technology. Strong verbal and written communication skills are essential.
  • Ability to work independently, manage stress effectively, and collaborate with colleagues in a team-oriented environment.
  • Capacity to handle multiple tasks and adapt to changes in workload and schedules while prioritizing responsibilities and making informed decisions.
