Job Summary:

As a key member of the Visiting Nurse Health System team, the Healthcare Transition Specialist plays a vital role in ensuring seamless transitions for patients from acute care facilities to home health care and/or hospice services. This position requires strong communication and coordination skills to serve as a liaison between healthcare providers, patients, families, and other organizations.

Key Responsibilities:

  • Conduct thorough assessments of patients' needs for in-home services and/or hospice care, providing written or electronic documentation as required.
  • Coordinate the initial delivery of home health care and/or hospice services, ensuring timely and effective implementation.
  • Serve as a liaison between Visiting Nurse Health System and facility staff, physicians, patients, families, and other organizations, fostering strong relationships and open communication.
  • Collaborate with Account Executives to develop and implement territory and account plans, driving referral and admission growth targets.
  • Work closely with referral partners, including hospitals, nursing homes, long-term care/assisted living facilities, physician offices, community agencies, and rotation partners, to achieve referral and admission retention and growth targets.
