Job Summary

We are seeking a skilled Healthcare Transition Specialist to join our team at Visiting Nurse Health System. As a key member of our team, you will play a critical role in ensuring seamless transitions for patients from acute care facilities to home health care and/or hospice services.

Key Responsibilities:
  • Admission Process Coordination: Collaborate with healthcare professionals to initiate and complete the admission process within acute care facilities.
  • Patient Needs Assessment: Conduct thorough written or electronic assessments of patients' needs for in-home services and/or hospice care.
  • Service Coordination: Coordinate the delivery of initial home health care and/or hospice services, ensuring timely and effective transitions.
  • Liaison Services: Serve as a liaison between Visiting Nurse Health System, facility staff, physicians, patients, families, and other organizations, fostering strong relationships and communication.
  • Marketing Initiatives: Collaborate with Account Executives to develop and implement marketing initiatives within designated territories, driving referral and admission growth.
  • Referral and Admission Targets: Achieve referral and admission retention and growth targets from referral partners, including hospitals, nursing homes, long-term care/assisted living facilities, physician offices, community agencies, and rotation partners.
Requirements:
  • Strong Communication Skills: Excellent verbal and written communication skills, with the ability to effectively interact with diverse stakeholders.
  • Collaboration and Teamwork: Proven ability to work collaboratively with cross-functional teams, including healthcare professionals, marketing teams, and other stakeholders.
  • Business Acumen: Strong understanding of healthcare business operations, including market trends, competitor analysis, and strategic planning.
  • Analytical and Problem-Solving Skills: Ability to analyze complex data, identify trends, and develop effective solutions to drive business growth.
