Hospice Nurse Case Manager

Transform Lives as a Hospice RN Case Manager
We're seeking a compassionate and skilled RN to provide exceptional hospice care to patients and families. As a Hospice RN Case Manager, you'll have the opportunity to make a real difference every day by upholding high standards of nursing practice.
Key Responsibilities:
- Maintain a specified case management load.
- Conduct initial and comprehensive assessments of patients and families.
- Participate in on-call duties as defined by the office on-call policy.
- Coordinate care among interdisciplinary team members and other healthcare providers.
- Evaluate and coordinate the need for services provided to the patient/family.
- Supervise and direct services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to the patient/family.
Requirements:
- Graduate of an accredited school of nursing.
- Active RN license in the state of South Carolina.
- One year of hospice care experience.
- CPR certification.
- Valid driver's license and auto liability insurance.
