Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will play a vital role in providing exceptional hospice care to patients and families, upholding our high standards of nursing practice.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs, including a history of current and previous illnesses, and a physical assessment of the patient, with emphasis on pain and symptom management.
  • Participate in on-call duties as defined by the office on-call policy.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, families, and attending physicians.
  • Coordinate care among IDG members and other healthcare providers to ensure continuous assessment of patient needs and implementation of the IDG plan of care.
  • Evaluate and coordinate the need for services provided to patients and families in accordance with the IDG plan of care.
  • Direct and supervise services provided by LPNs/LVNs, Hospice Aides, and Homemakers assigned to patients and families.
  • Facilitate consideration of alternative levels of care when indicated by patient status or needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • One year of hospice care experience.
  • One year of precepting or training experience preferred.
