Hospice RN Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and their families, ensuring their physical, emotional, and spiritual needs are met.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their hospice needs, including a history of current and previous illnesses, and a physical assessment of the patient.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), the patient, and the attending physician.
- Coordinate care among IDG members and other healthcare providers to ensure the continuous assessment of patient needs and the implementation of the IDG plan of care.
- Direct the services provided by and supervise the LPN/LVN, Hospice Aide, and Homemaker assigned to the patient.
- Evaluate and coordinate the need for services provided to the patient in accordance with the IDG plan of care.
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge.
- One (1) year of hospice care experience.
