Hospice RN Case Manager

Job Summary:
We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will be responsible for providing exceptional hospice care to patients and families, while upholding our high standards of nursing practice.
Key Responsibilities:
- Maintain a specified case management load and complete initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and other healthcare providers.
- Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of the IDG plan of care.
- Direct and supervise the services provided by and assigned to patients, including LPN/LVN, Hospice Aide, and Homemaker.
- Evaluate and coordinate the need for services provided to patients in accordance with the IDG plan of care.
- Facilitate the consideration of another level of care when indicated by patients' status/needs.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
- If a new graduate, 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a company-approved Preceptor Program.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
