We are seeking a compassionate and skilled Registered Nurse to join our team and make a meaningful difference in the lives of others.
Key Responsibilities:- Maintain a specified case management load, ensuring timely and effective care for patients and families.
- Conduct comprehensive assessments of patients' physical and psychosocial status, identifying needs and developing personalized care plans.
- Collaborate with the Interdisciplinary Group (IDG) to initiate and develop plans of care, making necessary revisions as patients' needs change.
- Coordinate care among IDG members and external healthcare providers, ensuring seamless delivery of services and achieving hospice care goals.
- Evaluate and coordinate the need for services, directing the work of LPNs, Hospice Aides, and Homemakers as needed.
- Facilitate consideration of alternative care levels when indicated by patients' status or needs.
- Graduate of an accredited school of nursing with a current RN license in South Carolina.
- Twelve months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
- One year of hospice care experience, with precepting or training experience preferred.
- CPR certification and valid driver's license with auto liability insurance.
