Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and their families, upholding our high standards of nursing practice.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care delivery.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status, identifying needs and developing personalized care plans.
  • Collaborate with the Interdisciplinary Group (IDG) to initiate and develop care plans, making necessary revisions as patients' needs change.
  • Coordinate care among IDG members and external healthcare providers, ensuring seamless delivery of services and achieving hospice care goals.
  • Evaluate and coordinate the need for services, directing the care provided by and supervising LPN/LVN, Hospice Aide, and Homemaker assigned to patients.
  • Facilitate consideration of alternative care levels when indicated by patients' status or needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with one (1) year of precepting or training experience preferred.
  • CPR certification and ability to provide proof of valid driver's license and auto liability insurance.
