Hospice Registered Nurse PRN On Call

Interim HealthCare Hospice is seeking a compassionate PRN On Call Hospice RN for weekend support starting Friday at 5:30pm till Monday at 8:30am. As the nation's first home care company, we are committed to helping patients live out their days in the most comfortable and meaningful ways.
Key Responsibilities:- Provide hospice patient care after-hours and weekends, as needs arise for our patients
- Enable patients to spend quality time with loved ones and participating
- Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient's plan of care.
- Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
- Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
- Educate family on the patient's prognosis, changes in care plan, medication and hospice goals.
- Take a holistic approach to care that engages the patient's mind, body, spirit and family.
- Registered Nurse (RN) license
- CPR certification
- Valid car insurance
- Prior Hospice experience 3+ years
- Strong critical thinking and problem-solving skills
- Excellent communication and interpersonal skills
- Proficiency in medical terminology and understanding of Hospice nursing care
- Ability to work independently and manage time effectively.
